Scheduling Appointments
To schedule a medical appointment, call the care center of your choice. For your convenience, we offer same day and advance appointments, and many of our locations offer evening or Saturday hours. Please click here for our appointment hours and driving directions.
To prepare for your first visit with your doctor, you should bring a copy of your health insurance card, a list of your current medications, your co-payment for an office visit and information about your medical history.
After-Hours Care
Because we know illnesses and injuries don't just happen during regular business hours, some of our locations offer evening and Saturday appointments for your convenience. If you experience a non-life threatening need after regular business hours, please call one of these locations for assistance.
An emergency situation is defined as a sudden illness or injury requiring immediate medical intervention. If you feel you are experiencing an emergency, call 911 or go to the nearest hospital.
Referrals to Specialists
If your doctor determines that you need specialty care not provided within Dominican Medical Foundation, you will be referred to a specialist. Services not available within Dominican Medical Foundation may require prior authorization. Your doctor’s office will coordinate this authorization process. Authorization decisions are based on your insurance benefits and whether services requested are medically appropriate.
