Dominican Medical Foundation is a division of CHW Medical Foundation (CHWMF), a nonprofit affiliate of Catholic Healthcare West. CHWMF operates clinics with locations (referred to as "care centers") throughout Northern California. With over 350 contracted physicians plus scores of allied health practitioners, CHWMF provides high quality, personalized care to its patients. CHWMF also provides health education and conducts medical research.
Dominican Medical Foundation provides health care services to Santa Cruz and surrounding area families through its professional service agreement with Santa Cruz Physicians Professional Corporation, the employer of the physicians serving patients at Dominican Medical Foundation care center locations.
