Dominican Medical Foundation, a service of CHW Medical Foundation (“Dominican Medical Foundation”), is a division of CHW Medical Foundation, a California nonprofit public benefit corporation that operates a California 1206-L clinic with locations (referred to as “care center locations”) throughout Northern California.
Dominican Medical Foundation provides health care services to Santa Cruz County area families through its professional service agreement with Santa Cruz Physicians Professional Corporation, the employer of the physicians serving the patients at Dominican Medical Foundation. Dominican Medical Foundation and Dominican Hospital are each affiliates of Catholic Healthcare West (CHW). CHW Medical Foundation also provides health education and medical research.
